I hereby give my permission for my son/daughter to attend the Van Hoosen 8th grade party on Thursday, June 4th, 2026 from approximately 10:30 am to 1:30 pm. I understand that the games and athletic devices may pose certain risks to the participant. I hereby assume such risks for the aforementioned student and waive and hold Rochester Community Schools, the 8th grade party committee and volunteers, and the company and its employees and operators of such said games and devices harmless from liability relating to the use of said games and devices. I also understand that my child must remain at the party until it ends. By checking the box, I agree to this waiver.
